As the SIL Coordinator, you will be integral in developing the team, bringing great care and support services to clients across the region.

The Role
As an RN Care Assessor, you will join the growing team of Registered Nurses supervising the provision of individualized care and support to our Participants, empowering them to lead fulfilling lives.

Your primary responsibility will include supervising support workers in both SIL and in-home support, ensuring the delivery of top-quality care to our valued Participants. Training and mentoring staff, learning everything about the client to ensure top notch supports are delivered by the team. As a support expert you will liaise, plan, participate and monitor care and support services to ensure all stakeholders remain engaged and up to date with support needs.

You will provide your clinical expertise in a variety of areas such as wound care, nursing assessments, continent management, PEG care and other clinical requirements as clients need support.

Skills & Experience
Able to build rapport and professional relationships with Participants, their support network and support staff
Able to plan, organise, coordinate, problem solve and analyse to a high standard
Current AHPRA Registration
Community/ Primary Health / Disability knowledge and experience
Highly organised
Able to work autonomously and act as a role model within a team, with a collaborative and cooperative approach.
Excellent communication skills
Sound computer literacy

Tasks & responsibilities
Establish, review and develop personalized care and support plans for our Participants, tailoring assistance to their unique needs and aspirations.

Oversee and guide support workers in the SIL and in-home support locations, predominantly around the Redlands, ensuring they deliver the highest standard of care and services.

Staff competency based training and supervision.

Roster management, on-call and shift coverage abilities.

Collaborate with the Engage Care team to create individual care plans that promote independence and well-being for each Participant.

Monitor the progress and development of Participants, making necessary adjustments to support plans as required.

Foster a positive and inclusive environment within ECS, promoting a sense of belonging and community among Participants, team members and the broader client stakeholder group.

Implement and uphold all relevant policies, procedures, and regulations to maintain the safety and welfare of our Participants.

Act as a liaison between Participants, their families, and other support services, fostering strong communication and partnerships.

Ensure Medication Management is compliant, as per medication management policy & procedure

Mandatory Requirements
Current AHPRA Registration (EEN/RN)

NDIS Screening check

NDIS Orientation

Valid driver’s license and an insured vehicle

Committed to ongoing learning and development

Working With Children’s Check- Employee (willing to obtain one)
